Self Management Support

 

Recognition for the importance of self-management is paramount to help support our patients. Individuals living with diabetes need to effectively manage it, on their own, without a clinician by their side at all times. Health care providers also need to understand how to better support, influence and build confidence in their patients` ability to self-manage. To better understand how to support better patient self-care, the Ontario Medical Association has published a background paper to support physicians in better understanding self-management. 

 

 

In Waterloo Wellington, the Regional Self-Management Program, offers many workshops for health care providers and patients to further their knowledge of effective behaviour change strategies.
