LifeLabs now offers my results which is a patient portal that provides patients with secure online access to most of their LifeLabs test results.  Most lab results are released from the testing laboratory and will be posted to patients’ my results accounts within 24 to 48 hours.  The my results website is available in English, French, Chinese and Punjabi.  For more information visit the LifeLabs website.

 

You can register for my results, by visiting myresults.lifelabs.com  within 30 days of your lab test. To register you will require the following information: valid OHIP number, active email address, date of your recent lab test (within 30 days of registration) and your unique lab visit number provided at the time of service.   

 

If you use Canadian Medical Laboratories (CML), you are unable to access your results on my results at this time. Lifelabs purchased CML Healthcare in 2013 and as CML locations are converted to “LifeLabs” Patient Service Centres, those patients will be able to register for my results. You are encouraged to ask your CML locations when this service will be available.

Local Pharmacy Support

Your local pharmacist can be a great resource and valuable member of your diabetes management team.
 

The MedsCheck Diabetes Program is an annual medication review by a community pharmacist for Ontarians living with diabetes. MedsCheck aims to provide education and optimize drug therapy. It is conducted in the community pharmacy in person with the patient or patient's care-giver. For more information contact your pharmacist.

Mental Health Resources

Symptoms of depression are more common in people with diabetes compared with the general population and major depressive symptoms are present in approximately 15% of people with diabetes.

 

Many Diabetes Education Programs have Social Workers on staff to provide counselling for those dealing with the effects of diabetes. For more information you can contact your local diabetes education program to inquire about access to counselling services.

 - Specialized Geriatric Mental Health Assessment: Provides comprehensive assessment of the older patient's mental health in order to provide accurate diagnosis and treatment options.

 - Specialized Geriatric Medicine Assessment: Provides a geriatric assessment, diagnosis, and treatment recommendations.

 - Community Responsive Behaviour (CRB) Team: Assesses people with cognitive impairment for specific responsive behaviours such as agitation, repetition, and wandering arising from dementia, mental health or addictions. Works collaboratively with caregivers to identify strategies to manage behaviour and reduce risks.
