COVID-19 Update

Please note, due to COVID-19, Diabetes Education Programs are now offering appointments virtually, either electronically or by telephone.  Some programs are seeing people in person, depending on the urgency.  Central Intake continues to accept and triage referrals.  Thank you.  If you have any questions, please call us at 519-947-1000 #283.

 

People with diabetes are considered part of the "high-risk chronic conditions group".  This means they are eligible for the vaccine in Phase 2 of the roll-out, which is currently scheduled to begin in April.  For more information, please click here.  

 

 

WELCOME

 

This site was developed by Waterloo-Wellington Diabetes and launched on November 14th, 2011 in recognition of World Diabetes Day.  The aim of the professional site is to be a resource centre for health care providers to support you in delivering diabetes care. 


Waterloo Wellington Diabetes, hosted by Langs in Cambridge Ontario, offers a central intake for all referrals to diabetes education and specialists.  This means improving access to diabetes care by finding the right care at the right time at the right place for individuals and families with diabetes.  Central Intake was launched in 2011 and has been very successful to date.  In August 2017, we launched eReferral for referrals to diabetes education or specialists. 

 

We also offer a regional Resource Clinician who supports health service providers throughout the region to deliver high quality, evidence-based care in diabetes.
