Pharmacy Partners

Your local pharmacist can abe a great resource and valuable member of your diabetes management team.

 

The MedsCheck Diabetes Program is an annual medication review by a community pharmacist for Ontarians living with daibetes. MedsCheck aims to provide education and optimize drug therapy. It is conducted in the community pharmacy in person with the patient or patient's care-giver. For more information contact your pharmacist.

Other areas where your pharmacist can help provide support and information, are:

  • Advice on your diabetes medications
  • Support with your insulin pen
  • Selecting a blood glucose meter
  • Safe "over-the-counter" medications
  • Sharps disposal

 

 

Community Pharmacy Outreach Programs (CPOP)

If you do not have a regular pharmacy or you are looking for specific information on diabetes, the Candian Diabetes Assocaition has created the Community Pharmacy Outreach Program, comprised of pharmacies recognized by the Association as providing exemplary service and a high standard of care in the area of diabetes management.

Another resource in the community is through pharmacists who have attained their Certified Diabetes Educator Certificate. These pharmacists have a marked interest in diabetes care and may be able to assist you with the management of your diabetes.
