COVID-19 Update

Please note, due to COVID-19, Diabetes Education Programs are now offering appointments virtually, either electronically or by telephone. Some programs are seeing people in person depending on the urgency.   If you would like to speak to a diabetes educator, or receive some education, please click the self-referral form in the box below. 

 

If you are experiencing symptoms or have been in close contact with someone who has it, please use this self-assessment to help determine how to seek further care. Click here for our COVID-19 Timely Topic information page.

 

If you have diabetes (Type 1 or 2), you are considered part of the "high-risk chronic conditions group".  This means you are eligible for the vaccine starting the week of May 10th.  You do not have to provide proof of your condition. For more information on when and where to schedule a vaccine, please click here.
