Most programs are seeing people in person, but may offer virtual appointments, either electronically or by telephone.  If you would like to speak to a diabetes educator, or receive some education, please click the self-referral form in the box below.

If you have diabetes (Type 1 or 2), you are considered part of the "high-risk chronic conditions group".  It is very important for you to get the Covid vaccine.  For more information on when and where to schedule a vaccine, please click here.
